Four are required — enough for me to quote accurately and know who I am replying to. The other two are genuinely optional, and leaving them blank is a complete enquiry.

What this form does not do
Three things it will never ask you for
A short form is a decision, not an oversight. These are the three things a longer version of this form would have asked for, and why each one is not worth your time.
No discovery call before I can help
The brief is enough for a fixed price. If I need to hear more, the free 20-minute call is the first thing I suggest — you will get an opinion on that call even if you never hire me.
No project-management account
There is no account manager and no ticketing portal. You email one person and that person builds it.
No follow-up sequence
If I say no, you will not hear from me again. If I say yes, you will not be chased by a salesperson, because there is not one.
